Continuity of Care Plus Program Resources
We are committed to supporting your efforts to provide the highest quality of care to our members. As a result, we are excited to announce that our Continuity of Care Plus (CoC+) program will continue in 2026, launching in February.
Carolina Complete Health is excited to continue the "Tuesdays at Two," a Continuity of Care Plus (CoC+) Information Session. Please join us each week to learn more about what’s new for 2026 and how you can use the CoC+ program to support quality continuity of care for our members.
This is held every Tuesday from 2-3 PM.
